SJ Health accepts many health programs such as Medicare, Medi-Cal, and Family Pact.

SJ Health is part of both the Health Plan of San Joaquin and Health Net Medi-Cal provider networks in San Joaquin County. *We can help you select SJ Health as your provider if you are new to either health plan, or if you want to change your PCP.

We also offer a “Sliding Fee Scale” for primary care services for our patients without health coverage or insurance. This means the amount you pay will based on your income

Sliding Fee Scale

What is it? Our Sliding Fee Scale is a payment program that allows eligible patients to pay for health services at a discount.
Do I qualify?
To see if you qualify for a Sliding Scale Fee we need information about you, including:
- o Your household income (before taxes).
- o Your family size.
How long will I be on Sliding Fee Scale?
You can remain on Sliding Fee Scale if you continue to qualify.
- o You will fill out a patient eligibility form at your first visit and again annually.
- o You must notify if there are changes to your income and/or family size.
How much will I pay?
Sliding Scale fees differ depending on your income and family size.
- o There are four fee levels of co-payments ranging from $30-60.
- o After your eligibility is assessed, you will be assigned one of the four fee levels.
